Discharge and AftercareDischarge / Return Home

Aftercare is crucial for your child’s ongoing recovery.

We believe strong outcomes depend on continuity and collaboration. From admission through discharge, we coordinate closely with your teen’s existing providers and aftercare programs — whether that’s our IOP or another trusted outpatient partner. Our team stays in regular communication with the broader care network to support timely transitions, shared understanding, and a clear path forward.

From the moment your child is admitted to Muir Wood, your treatment team will begin planning for aftercare services. Preparing for your child’s return home early on ensures that everything is in place when it’s time for discharge. This may include options like an Intensive Outpatient Program (IOP), Partial Hospitalization Program (PHP), a private therapist, family therapy, or other services.

Muir Wood believes that ongoing parent support after discharge is key to family recovery. We highly recommend participating in the Parent Aftercare Class (free for all alumni families; see additional information below) and continuing in the weekly Parent Support Group for both current and alumni families. These programs are part of our comprehensive family programming model, which provides structure and support at every stage — from your teen’s first week in treatment to long after discharge.

Your treatment team will provide guidance and recommendations for aftercare services. However, it is important for you to work with them and take the necessary steps to arrange your child’s continued care after discharge. Our team will help with referrals, send records, and arrange virtual intake sessions when needed.

It is the parent’s responsibility to participate in planning by reaching out to programs to ask questions, signing release forms to share information, and following up on the steps needed to complete the acceptance and admission process.

Parent Feedback Survey

Your feedback is important to help us improve our services for future families. You will receive an email invitation to complete a survey at different times during your child’s treatment. The first survey will be sent 10 days after your child’s admission. The second survey will be required on the day of your child’s discharge. After treatment is complete, we may send additional surveys to track long-term outcomes. We try to keep these surveys short and focused on the most important information that helps us improve our support for adolescents and their families. Your honest feedback is greatly appreciated!
